Enrollment Form

Please complete and submit the enrollment form below. Information on the enrollment form marked by an asterisk is required. If a question does not apply, please write 'none' or 'n/a' so that the form is not returned to you. If the form is returned, please scroll down to see what information you need to provide.

Once you have submitted the form successfully, an automatic enrollment confirmation message will be sent to the student email address you have entered. You should receive this confirmation within a few minutes of submitting the form. If you do not receive a confirmation message, please check the email address you entered and resubmit the enrollment form.

Don’t forget to return to the other items on the Enrollment Checklist after you have submitted this Enrollment Form!

Minnesota Online High School is open to any Minnesota resident, age 20 or younger, who is ready for 10th, 11th, or 12th grade high school classes.

As stated in the Minnesota Online High School Enrollment Policy, “Minnesota Online High School does not limit admission to pupils on the basis of intellectual ability; measures of achievement or aptitude; or athletic ability and does not discriminate on the basis of race; color; creed; religion; national origin; sex; gender; marital status; disability; status with regard to public assistance; sexual orientation; age; intellectual ability; measures of achievement or aptitude; or athletic ability.”

IMPORTANT NOTES: In order to properly use the MNOHS learning management system and online text books, you MUST 1) have a Windows computer, 2) use Windows XP or Vista as your operating system, and 3) use Internet Explorer 6.x or above or Mozilla/Firefox 2.x or higher as your browser--not AOL. A high-speed internet connection is highly recommended! Before you begin the enrollment process, take a few minutes to review the hardware and software requirements and check your computer to make sure that you have installed all of the required software. All required software is free.

Parent or Guardian Information
Students age 17 and younger must have parental permission to enroll at MNOHS, and staff will be in contact with your parent.

If you are over 18, or if you turn 18 during the academic year, we will assume that MNOHS has permission to contact your parent unless you state otherwise below.

If you are over 18 and receiving special education services, MNOHS will not contact your parent without your permission.

NOTE: Parents and other family members can be an important source of support for online learners. Please consider carefully before deciding that you do not want MNOHS to contact your parent. If you want to change the decision you make, you may contact MNOHS to do so.

Please Note: if you will be enrolled at another high school at the same time as you are attending classes at MNOHS, you must print a "Supplemental Notice of Student Registration" form. This must be signed by both your high school counselor and your parent (if you are under 18) and must be returned to MNOHS before you can start classes. Your enrollment will not be complete until we receive this form. If you are only going to be enrolled at MNOHS, you do not need to complete the supplemental/"dual" enrollment form.

SPECIAL EDUCATION NOTE: If you have been receiving special education services, please click here to review the Special Education section of the MNOHS website to learn about the services available. You may contact the MNOHS Special Education Director at special.education@mnohs.org to discuss how to meet your individual special education needs. If you enroll in MNOHS, please request to have your special education records (most recent IEP and Evaluation Report) sent or faxed to the MNOHS office immediately so we can work with you to plan your special education services.

Students enrolled at MNOHS ONLY will receive assistance selecting classes to meet their remaining graduation requirements, but you may also state your preferences. Please list any courses that you are especially interested in taking here:

Previous School Information
