Online Application Form - Supplemental

If you are applying as a Supplemental Online Learner (OLL), the MNOHS application for enrollment has two parts:

  1. this application form, and
  2. a signed Online Learning Form (OLL) to be completed by your family and your "bricks-and-mortar" school's counselor that confirms your application.

To confirm and hold your place as an OLL student at MNOHS, you must complete and submit both parts.

IMPORTANT:
The application windows will close on the following dates:

  • Quarter 1 | Starts August 30, 2012 - Window closes on September 14, 2012.
  • Quarter 2 | Starts November 6, 2012 - Window closes on November 16, 2012.

OLL students WILL NOT BE ENROLLED in supplemental course(s) at MNOHS unless your application is completed and confirmed by these deadlines.

To be able to access the full MNOHS curriculum, you must have access to:

  1. a computer
  2. a high-speed internet connection

 

Minnesota Online High School is open to any Minnesota resident in grades 9 through 12. As stated in the Minnesota Online High School Enrollment Policy, Minnesota Online High School does not limit admission to pupils on the basis of intellectual ability; measures of achievement or aptitude; or athletic ability and does not discriminate on the basis of race; color; creed; religion; national origin; sex; gender; marital status; disability; status with regard to public assistance; sexual orientation; age; intellectual ability; measures of achievement or aptitude; or athletic ability.
