Print, sign, and mail or fax the "Enrollment Confirmation and Learning Support Agreement " within one week of submitting the online enrollment form.  This confirms your intent to attend MNOHS.  Your enrollment is not complete without it--if you don't submit it, you could lose your place at MNOHS and might need to go on the waiting list. 

If you are under 18, we require a parent or guardian's signature as well. 

Also included with the enrollment confirmation are parental permission and release forms for science and PE.  Although you don't yet have your course schedule, we ask that you return these signed forms to us now so that you won't experience any delays in starting your courses.  If you are over 18, please sign these forms yourself.
